Might be something going on with TTV LOL as well. I use Chrome and had both the twich adblock extension along with TTV LOL and was getting the "Commercial Break In Progress" screen on everything, including channels I had a sub with. Disabled TTV LOL and kept the others enabled, restarted chrome and tried again and ads were gone.
