As time goes on and your fort grows, your game will start to slow where everything will feel slower. That's just the nature of things. However, there is inherent slowness that you create with a bigger fort. Bedrooms are probably farther away from the task. Temples/Taverns, will eat into a dwarf's productivity sometimes.
