I legit run straight into the middle of the room and kill prime while tanking the other two but no achievement. You probably need to do damage two the other two to be considered "under attack". I just tried that but still nothing. Well idk what else it could be. Maybe you still have to kill all three but prime first.

Melee hits instantly while mage attacks take time to travel. If rex dies while he's moving then the death animation adds like 2-3 ticks of delay before it dies, so you really just have to time the melee hit on prime then. I used to do this regularly. If you leave prime alive but crit with venom stacking on him, than you crit rex from the safespot.
