As for your first issue, bladed dive will only reset if you kill the your primary target. So what is happening is you're bding killing everything except the primary target and your bd isn't resetting. Just hit it with a cleave or a basic asap and it'll still reset your bd. There's nothing really buggy about it, just a small learning curve.
