Currently, on live servers, you need to transmogrify a specific Artifact weapon to get the forms you want. Pretty sure I have unlocked the additional appearances as I can access the additional druid forms and the artifact forge shows I have unlocked the additional appearances, I just don't seem to be able to access the actual weapon appearance corresponding with the druid form.
